• Cancellations: Cancellations or rescheduling requests must be submitted at least 24 hours prior to the scheduled slot. Failures to notify us within this window may result in a cancellation fee.

4. Billing & Financial Agreement

Payment for all treatments is required at the time services are rendered.

• Insurance Claims: While we assist in filing insurance claims on your behalf, you are ultimately responsible for any fees, deductibles, or copays that your insurance provider fails to cover.
• Cost Estimates: Any treatment cost estimate provided by our staff is an estimate based on initial diagnostic reviews. Actual treatment costs may change if clinical discoveries require modifying the treatment plan during the procedure. We will discuss changes before proceeding.
